Comparison of Laparoscopic Cholecystectomy with Open Chelecystectomy



Abstract
Laparoscopic cholecystectomy has become the golden standard method of treatment for many patient with symtpomatic cholellthiasis. Since laparoscopic cholecystectomy (LC) was performed first in France, in 1987 by Philippe Mouret, great interest
has
been
genrated by the recent development of LC. We compared the 130 patients who underwent laparoscopic cholecystectomy at Marysknoll hospital from Feb. 1992 to Mar. 1994, to the 79 patients undergoing standard open cholecystectomy (OC) during
pre-laparoscopic cholecystectomy era. Exclusion criteria initially included previous upper abdomonal operation, severe acute cholecystitis, choledocholithiasis, and inability to tolerable gneral anesthesia. The first two of these are now only
relative
contraindications with increased experience.
@ES The results of the study were obtained as follows;
@EN 1) Age distribution were similar between two grous. The mean age of LC group was 47.7 years and that of OC group was 50.8 years. The male to female ratio of LC group was 1:2.3 and that of OC group was 1:1.6
2) The shorter opertive time (mean 92 minutes versus 99 minutes, p<0.05) was required to perform laparoscopic cholecystecotmy than standard open choecystectomy.
3) There was 22 complications (kincluding one bile duct injury, 16.9%) in the LC group and 13 in the OC group (16.5%). There was no statistically significance.
4) Seven of the 130 LC procedures (5.4%) required conversion to an open cholecystectomy.
5) The patients who underwent LC were discharged from the hospital (mean 4.3 days versus 8.4 days, p<0.05) much sooner postoperatively than those treated by standard open cholecystectomy.
Laparoscopic cholecystectomy offers the majority of patients with symptomatic cholelithiasis as an improved treatment option, resulting in significantly less postoperative pain, shorter hospital stays. better cosmetic result, and more rapid
return
to
normal activity than standard open cholecystectomy.
